SOP's For CDE Activities
University Medical & Dental College (UMDC) Faisalabad is regularly organizing CDE activities for faculty members and medical officers. These activities are organized by Department of Dental Education – CDE office. CDE office keeps a complete record of all activities undertaken by faculty members and doctors working at UMDC. All CDE activities organized by UMDC need to be registered by the University of Health Sciences, Lahore for grant of credit hours.
  • Any department wishing to organize a CDE activity will write to DME/ CDE office for award of credit hours by UHS at least 40 days prior to activity with soft copies of the following documents

    1. Duly filled application form (available at UMDC website)
    2. CVs of all facilitators in a zipped folder
    3. Program of activities
    4. List of registered participants

  • The organizing department will provide the following documents to the corresponding coordinator immediately after the activity for countersigning & dispatch to UHS.
  • Hard copies of attendance sheet(available at UMDC website)
  • Feedback forms (available at UMDC website)
  • Soft copy of final list of participants & their PMDC registration number
  • CDE office will be responsible for liaison with UHS regarding information of activities, attendance of registered members with feedback, facilitation of observers and audit
  • CDE office will keep a record of all these activities and issue a certificate carrying total credit hours earned by the registered members
